Questions in this template, and why they matter
| Question | Type | Why ask it |
|---|---|---|
| How many bedrooms?(required) | Number | Bedroom count is the first input for how long the clean takes and what to charge. |
| How many bathrooms?(required) | Number | Bathrooms take the most time per room, so they move the price more than anything else. |
| How often would you like cleaning? One time · Weekly · Every two weeks · Monthly | Radio | Recurring clients usually get a lower rate and a set team, so frequency changes both price and staffing. |
| Do you have pets? | Yes / No | Pet hair adds time, and the team needs to know about dogs before they open the door. |
| Any add-ons?Inside the fridge · Inside the oven · Interior windows · Inside cabinets · Baseboards · Laundry | Dropdown (multi) | Add-ons change the time on site, so the schedule and the quote both need them before the visit. |
| How will the team get in?Someone will be home · Lockbox or door code · Key left for us · Other | Radio | Knowing the entry method ahead of time avoids a locked door and a lost slot. |
| Photos of areas that need extra attention | File upload | Photos help you quote move-out and post-renovation cleans that don't fit the usual room count. |
| Anything else we should know? | Long text | Parking, allergies, rooms to skip, and product preferences all belong here. |

Tips for a better cleaning request form
- In ServiceReserve, use Multiple amounts for bedrooms and bathrooms; each counter can carry a price, and Show total amount lets clients see the estimate as they go.
- Put prices on add-on options so an inside-the-oven request adds to the total instead of turning into a surprise on the invoice.
- Keep the form short for repeat clients by attaching move-out and post-renovation questions to those services only in Services.
- Use Morning / Afternoon or Time Windows in Schedules, and set minimum notice so you have time to staff a deep clean.
- If you quote before you book, leave scheduling off on a separate form and use it as a cleaning quote request form.

Frequently asked questions
Can the form show a price based on bedrooms and bathrooms?
Yes. Use Amount or Multiple amounts counters with a price per bedroom and bathroom, then turn on Show total amount. The client sees the running total before they submit.
Can I use it only for quote requests?
Yes. Leave the scheduling step off and the form collects the home details without a date. You follow up with a quote from the request.
Can I collect entry instructions and property details?
Yes. Along with your own questions, the address step can collect entrance information and whether the client owns or rents the home.
